It may be seen in cerebellar disease, possibly as a reflection of the kinetic tremor and/or the impaired checking response seen therein (cf. An arm so affected may show apraxic difficulties in performing even the simplest tasks and may be described by the patient as uncooperative or ‘having a mind of its own’ (hence alternative names such as anarchic hand sign, le main étranger, and ‘Dr Strangelove syndrome’). These phenomena are often associated with a prominent grasp reflex, forced groping, intermanual conflict, and magnetic move ments of the hand. Different types of alien hand have been described, reflecting the differing anatomical locations of underlying lesions. Anterior or motor types: Callosal type: characterized primarily by intermanual conflict. A paroxysmal alien hand has been described, probably related to seizures of frontomedial origin. Visceral mus cle is found in the respiratory and digestive tracts, in the blood vessels, and in the eyes. A person with a mild strain will experience local pain with contraction that decreases with stretching, some decrease in range of motion, point tenderness, mild loss of function, and possible muscle spasms. A moderate strain will be more painful than a mild strain when the muscle is con tracted and/or stretched. A snap or a tearing sound is sometimes heard by the patient when a moderate strain occurs, and delayed discoloration of the tissues may also occur (see Figure 14-22). There will be point tenderness, moderate swelling, moderate to severe loss of function, and possible muscle spasms. There will be a decrease or increase in range of motion, severe loss of func tion, possible muscle spasms, and a defect in the muscle that is palpable. If pain is present with a severe strain, there will be no change in the degree of pain when the muscle is contracted or stretched, because the muscle is com pletely torn (see Figure 14-23). In addition, observe the amount of initial swelling to the injury to establish a baseline guide for gauging how well the injury is responding to treatment. Treatment for a severe strain requires ice, compression, and elevation as do the other types of strains. In addition, immobilize the affected area, make the athlete as comfortable as possible, and activate the Emergency Action Plan. The athlete may require surgery to repair the damage and may be unable to return to play for as long as one month to one year. Follow-up Treatment: Perform static stretching to maintain range of motion by lengthening the muscle to the point of tightness or pain—then hold that position for 15 seconds. Repeat this procedure at least three times a day, stretching the muscle morning, noon, and evening. Bring to their atten tion different ways that proper mechanics can prevent injuries. Fibrous: (immovable) includes the bones of the cranium, or skull tw o or m ore bones m eet. Mobile joints are the most frequently injured joints and are grouped according to the way in which they work. For example, pivot joints allow rotation on a single axis, and hinge joints allow the adjoining parts to flex (bend) and extend (straighten). A pivot joint in the wrist permits the palm of the hand to turn upward or downward. Hinge A joint in which the two surfaces are molded together closely, allowing a wide range of flexion and extension along a single plane. Saddle A joint in which two surfaces, one convex and the other concave, fit together. Condyloid (ellipsoid) A round or oval end of a Gliding Two facing bone surfaces meet, allowing bone fits into an oval cavity, allowing all types only gliding movements. This type of joint and the ankles as well as the vertebrae in the can also be found in the metacarpals. The joint is enclosed in a band of w hite, fibrous, a protective capsule that contains synovial fluid. This fluid is colorless and contains connective tissue that mineral salts, fat, and other substances. A bursa is a sac full of synovial fluid that reduces friction between tendons, bones, ligaments, and other structures. A meniscus is a tendon cartilaginous disc surrounded with fluid that also reduces friction during move ment and adds stability. Soft tissue can withstand some compressing force; however, if the force is excessive and not absorbed, a contusion will occur. Tendons and ligaments are designed to effectively withstand tension forces, but do not resist sheer or compression forces well. Excessive tension or sheer forces cause injuries such as ligament or capsular sprains or muscle strains with varying degrees of severity. A sprain is the overstretching sprain and/or tearing of ligaments or other a stretching or tearing of connective tissues caused by traumatic the ligam ents, character twisting of a joint (see Figure 14-26). A ized by the inability to sprain may also involve the articulating m ove, deform ity, and pain. In general, symptoms of a sprain include deformity, crepitation, point tender crepitation ness, and immediate swelling. When stressed form a joint as it m oves for evaluation, a moderately sprained joint will exhibit some laxity when compared through its range of to the uninvolved side, indicating instability. A third-degree or severe sprain is a complete tearing of the ligaments and/or articulating capsule. When stressing the ligaments for evaluation, an opening of the joint that appears to have no endpoint will be found. While applying stress to a joint injury, if it seems the injured ligaments are offering no resistance, then there is no endpoint— no resistance by the ligaments to the pressure being applied—and a third-degree or severe sprain is indicated. This provides information about what type of appearance and movement to look for in the injured side. Knowing what to look for is important because there will be only one opportunity to check the involved side—the athlete won’t let the examiner do it again. For example, if the ath system ic steps taken to lete suffers a sprained ankle on the field, have the athlete avoid putting m itigate or m inim ize injury: pressure on the injury while walking off of the field. To accomplish this, protect, rest, ice, have two or more people support the athlete or have the athlete use com press, and elevate. Ice the surrounding tissues immediately for 20 minutes, followed by an hour during which the ice is off. Taping and wrapping procedures for foot and ankle ligament sprains can be found in Chapter 21. Once tissues have recovered, strengthen ing should increase to isotonic exercises. Braces can add external support to damaged ligaments while the injury heals (see Figure 14-28). Prevention: Check the event area for possible hidden dangers such as holes on the field, water on the court, etc. Also, instruct the athletes in the appropriate strength ening exercises for the areas of the body that are vulnerable to injury, depending on the sport being played. Appropriate taping or bracing of vulnerable body parts can provide additional protection (see Chapter 21). Dislocations and Subluxations A dislocation is an injury resulting from a force that causes a joint to go beyond its dislocation normal anatomical limits. These symptoms can be detected by subluxation comparing the injured shoulder to the uninjured one. Signs and symptoms of a sub luxation include dead arm weakness (the inability to lift the arm) and pain. Recheck the area below the injury for pulse and sensation to make sure that nothing has been made worse by the movement and splinting. If the weight of the ice causes the athlete more pain, reduce the amount of ice being applied. If this happens, the urgency to see a physician is not as great, but still needed. Prevention: Shoulder braces will decrease external rotation and abduction of the shoulder girdle (a common point of dislocation).
